Patient Care Management


Patient Care Management’s team features Tim Feist, Vice President of Performance Improvement, and Medical Directors Christopher Snyder, DO, and Edward Klopp MD, three patient care management assistants, four social workers and 25 registered nurses. 

Hospital based case management is an exciting and evolving advanced practice. The patient care coordinators and social workers in our Medical Center are experienced and creative professionals with a strong work ethic and an admirable commitment to patient advocacy and quality improvement. Their daily work is a collaborative effort with all caregivers internally and the caregivers in our community. They are involved with safe discharge planning, utilization review, social issues and prescription assistance.  Throughout a patient's experience they are responsible for the appropriate utilization of resources and access to care.

There are either patient care coordinators or both patient care coordinators and social workers assigned to each patient care floor. A team approach has been developed between interdepartmental staff, the floor nurses and physicians assigned to the patients, family members and the patients themselves.

Patient Care Management continues to strive to meet our patients’ needs in a timely and efficient manner, while aiming for complete customer satisfaction.
